Before I moved, the local hospital started doing the same type survey -
only it was at two weeks.  If you use volunteers, I think it would be wise
to have a support staff person who the volunteer can refer any moms with
questions/problems that need to be called back.  I always was interested in
the questions related to what helped at the hospital to get breastfeeding
started, what was not helpful, who they have gone to with questions about
breastfeeding - friend, parent, spouse, obstetrician/pediatrician/family
doctor, midwife, LC, hospital nursery staff, LLL?  You could also ask them
when they decided to breastfeed to follow up on that earlier thread we had
on lactnet.
If that hospital is still on lactnet, maybe they will respond with a copy
of their questions to you.
