Dear Katherine -
It is very common to experience breast tenderness while nursing during
pregnancy.  For some mothers it will go away, often during the second
trimester, though for other mothers it may last through the whole pregnancy.
 It varies from one pregnancy to the next.  Reminding the toddler to stick
out their tongue and to nurse gently may help.  As the milk supply is reduced
toddlers may suckle harder in an effort to get more milk.  Mom may want to
limit the toddler's nursing time at the breast.  Some mothers may find they
just don't like being touched on the breasts at all and may encourage the
toddler to wean. You might recommend the book "Mothering Your Nursing
Toddler" and the booklet "Nursing Two Is It For You?" both available through
La Leche League.
