I don't have a ref. but I would have both the neonatologist/pedi AND the
mom's OB/HCP write letters. The neo can say that human milk is the optimal
fdg and the pump is needed to supply it. Babe is at high risk for infection,
etc. (Is he a premie?). The Ob can say mom will develop mastitis without the
pump. Also have the dr's write "XXX brand name electric pump and kit" on a
prescription sheet and turn that in to the ins. co. When I was an nicu
nurse, we had a standard letter and the neo would sign it.
