Hi all,
I have seen this mom once when baby was 10 days old - he is now 4 mo old. I
have talked to the mom  once or twice a month since, and things have always
been a struggle with this baby. I will probably see this mom again later this
week since she is at her wits end.

Baby at 10 days seemed to have partial tongue tie and mom had flat nipples.
Nursed one side only at each feed with help of a nipple shield was in the 75%
in weight. Nursed every hour from 6-12midnight.

 Weight checks  by 2mos showed baby gaining but dropped in percent to 50%.
Mom still using shield.Baby fussy and talked about diet related causes. Baby
nurses every 1 1/2 hr. but sleeps 6 hr at night.Doctor thought tongue-tie was
minor and won't do anything about it and mom doesn't want to press it.

At 11 weeks mom called about colic behavior and eczema on baby's face and
knees. Nursed every 1 1/2 hr on one side only still with shield. Now mom may
try to eliminate chocolate and dairy from diet.Stools green.

At 14 weeks Stools changing to yellow but sometimes orange. She will try
nursing on both breasts more frequently . Baby will nurse without shield but
mom still uses it if baby gets fussy at breast. Baby very fussy recently,
talked about food related, teething, ear infection (pulls at ear) Mom still
eats cheese . Last weight check showed average gain 6 oz./week. Still nurses
every hour with 1 3hr. nap and sleeps 6 hrs. at night. Recommend she see
allergist for baby eczema.

At 4mo. weight check showed gain but baby now in the 25% doctor not
concerned. Mom still using shield probably more than not especially in
evening when baby pulls off breast frequently and is fussy. She says with
shield he is calmer. Still eats 12x/day and sleeps 6hr. at night. Rarely
content enough to be put down. She started some solids but finds it hard to
find a time when baby is happy enough to try (he prefers nursing when he is
hungry and he eats every hour!) Mom is highly frustrated, her husband still
does all food shopping and laundry since she is either feeding or comforting
baby.

I have asked to see baby again I suspect moms supply needs to be improved by
pumping and I think baby needs more to eat. She still hasn't seen an
allergist (eczema still present) She will try to use shield less (we talk
about this every time she calls) . Any thoughts on this case would be
helpful. I have seen a change in weight percentile occasionally  without
concern but this baby doesn't sound happy and mom certainly is not happy
although she seems slow to initiate change (it usually takes a week or two
before she gets around to recommended changes ie. diet).
