Waterbeds are used in all the nursery isolettes in Sweden and Norway (I did
an internship in Norway). Their philosophy is that waterbeds are much more
soothing, and I believe it. Anyway, the babies are always put on their
backs. Both of our children has slept with us in our waterbed--on their
backs until they managed to roll over. However, rolling over in a waterbed
is much harder for them because the bed "gives." So, for those who have
early rollers, this is an advantage. We set up a short-shorn lamby
underneath them and put rolled receiving blankets (taped to stay rolled) on
either side. Those rice-bag gadgets to position a baby will also work for
a limited time.
