Can you believe "I" am a bit behind in my reading???
Anyway, I had to respond to this with my own personal observations.
As a mom who pumped her milk for 4 months while working to get her babe back
to breast (luckily after only 2 mo) I have to say that the foremilk hindmilk
controversy should still be discussed.
Many is the time when I was only able to pump for 5 minutes, and then had to
go back and finish a bit later.  I always used seperate bottles.  The milk
pumped first had very little "cream" or "fat", and the 2nd bottle was always
FULL of the "cream".  I have a friend who pumped for her daughter at my house
one day.  She only had small bottles to use.  The bottles that were filled at
the beginning of the pumping had a tiny bit of cream, the bottles filled at
the end were nothing but cream.  A mom I worked with this past week (my first
home visit!!!) had obvious foremilk leaking , a nice cloudy clearish fluid at
the first letdown, but as she continued to leak (for the next 45 minutes!) it
became a thick (and I mean thick!) creamy golden yellow!  Tells me there is a
big difference as to where the foremilk hindmilk are in the breast and during
the feed.
How about one more situation to add to the fire:  We all know that if a baby
only BF a minimal amount of time before being switched to the next breast she
can end up with a foremilk hindmilk imbalance causing gassiness, fussiness,
and green poopys.  Put the baby at one breast per feed for a few days (to get
the hindmilk) and we start to see a change.  Baby goes back to normal poopys
and the gassiness and fussiness go away.
So where does this leave research?  Where does this leave the final decision
on the foremilk hindmilk debate?  Who knows?
But for me, there is no debate.  There is a difference in where the two types
of milk are in the breast, and in some moms there is a difference of how much
of each type they may be producing during different times of the day.
