I would agree with Dr. Hale, that for theoretical reasons, sertraline
(Zoloft) is a better choice than fluoxetine (Prozac). However, we
have had many mothers on fluoxetine without any discernible effects on
the baby without any use of abm. Another antidepressant which should
be theoretically superior to others is nefazodone (Serzone).
(Estimated systemic bioavailability 15 to 23%, decreased availability
with food, e.g. milk, large volume of distribution, protein binding is
reported to be >99% bound to protein). Food decreases absorption by
about 20%. The metabolites, though active, are less so than the
original drug.
