Here's one from a book titled "Take Charge of Your Child's Health" by
George Wootan, MD, and Sarah Verney, page 72:

        1 tsp sodium chloride (table salt)
        1/2 tsp sodium bicarbonate, or baking soda
        4 tsp potassium chloride (salt substitute)
        1 Tbsp sugar
        1 quart water

        be sure to use measuring spoons, not eating utensils for accuracy.

I have not used this formula personally, but I have successfully used his
advice for nursing a baby who is vomitting excessively.
