Assuming that the problem may be maternal:
Any possibility of retained placenta? (probably unlikely with a CS)
Did she experience breast changes during pregnancy?
Any history of endocrine problems such as thyroid?
Any past injury to spinal column?
Any breast surgery including cosmetic?
Any past head injuries or problems such as brain tumors?
Any postpartum hemorrhage?
These are the reasons that come immediately to mind. I'm sure there are others.
Sounds like this mom needs a supportive HCP who would be willing to order
some tests and scripts for her (e.g., trying Reglan or oxytocinon, ordering
prolactin testing, a TSH screen, etc.) I'm sure that the stress of being
accused of starving her baby is further compromising her let-down reflex.
