I'm sorry this mother didn't come to you sooner!

I agree with your treatment plan ideas.  I wonder if they'd be willing to
culture the fluid from the penrose???

I haven't seen Parlodel used in a long time.  I've seen a nasty rebound
feature a couple of weeks after the drug is given:  engorgement, leaking,
etc.  Why did they bind this poor woman?  I would definitely have her pump
with a hospital-grade pump, just one side at a time for comfort.  If the
suction is too much on the bad side, you may want to try a pump with an
adjustable speed.  I have found that loosening the cylinder a little bit can
drop the suction to a more comfortable level for moms in really bad shape.
 Let her know that she may have quite a bit of milk leakage from the wound
site. It can be very disconcerting and messy.

Is she still willing to breastfeed?  Good for her!!!

Please post the outcome of your talk with the surgeon.  Inquiring minds...
