Regarding the mother with the very large nipples: I have worked
with two mothers who had nipples that were so large that it was impossible
for the baby to reach the milk sinuses. Both babies were third or fourth
babies who were failing to thrive. In both cases, earlier babies had done
fine, but mom's nipples had stretched and become larger with each baby.
These moms were both very committed to breastfeeding with a long association
with LLL. Both were able to breastfeed using a combination manual
expression at breast and a nursing supplementer and pumping with a large size
flange. With mom number one, baby was able to breastfeed without
supplementation around 4 months, as were subsequent babies. Mom number 2
is returning her breastpump today after 5 1/2 months, since baby is finally
able to nurse and thrive without supplementation! Obviously this took a
great deal of commitment.
