Marie,
Could this be pus? If the breast is that painful I would wonder about
a breast abcess. The last two women I have seen with abcess have had
surprisingly few systemic symptoms eg no fever, just felt tired and
had some breast symptoms such as pain, redness or swelling.
One mother only picked up there was something wrong when her milk
looked 'greenish' and her hubby said it tasted salty!!
An ultrasound would be a non-invasive way of telling for sure.
