Patricia - I wonder if one or more of the nipple pores could be partially
blocked, leading to backup into the sinus.  I have seen this a few times
with badly damaged nipples.   If this sounds right, lots of soaking may be
in order (preferably accomplished by having mom lower breasts into a basin
of warm, salty water).
