We discharged a breastfeeding mother yesterday on her 3rd post-op day
following a primary C-section for failure to progress.  Pt. had a
history of PIH (pregnancy induced hypertension) and her post-op course
was complicated by a "spinal headache".  Today, she was readmitted to
the intensive care unit with seizures!  My question is med related.
Currently the Dr. has her on Dilantin 100mg p.o. TID,following a 1.5
Gm loading dose. She is also on Synthroid .2mg daily (had been taking
.3mg at home), and Capoten 25mg p.o. TID which is a new med for her.
The Dr. wrote an order to BIND HER BREASTS!  The nurses in CCU
investigated further and want reassurance of the drug levels in EBM.
The Dr. will agree to pumping but his main concern is the Dilantin.
My sources say "insignificant amounts in breastmilk and generally
considered safe".  The only info I have quick at hand for Capoten is a
milk/plasma ratio of o.o12.  I am aware of the use of Synthroid.  If
you have further info or info differing from this, please respond.
I GREATLY appreciate the expertise here on Lactnet and the willingness
to share and help one another.  I should add that the baby nursed well
when mother was comfortable enough to feed with the head pain.  Thank
you!
