re the depressed mom of the preemie - what about psychotherapy ?- up
to daily if necessary, but likely twice a week - using cognitive
therapy or supportive techniques, instead of medications. If drugs
are necessary, zoloft is still the only one with a case report of no
levels in the infant. For any drug, get drug levels from the infant
to show the neonatologist what's going on. Also, read the article in
American Journal of Psychiatry, September 1996, vol 153, #9, page1132
by Wisner et al about antidepressant treatment during breastfeeding -
especially the conclusion section - it's a wonderful discussion of
risk/benefit ratio and informed consent.
