I have never worked with a mom in this situation but here are some
suggestions based on what I know about celiac disease.
1. It is true that any amount of gluten in the diet can cause someone to
have a relapse. How is the baby doing on gluten free diet and mom
breastfeeding without resticting her diet?
2. If baby asymptomatic would not worry about changing mom's diet IF the
small bowel biopsy is normal. Usually a first biopsy is obtained to make
the diagnosis and a second one to document the remission. If the biopsy
is normal on breastmilk with mom eating gluten, I wouldnt worry.
3. If the baby is symptomatic and or the biopsy is abnormal would
consider gluten free diet for mom too.
4. Gluten free diet is VERY restrictive. Usually patients and parents
belong to support groups through which they learn about the diet and
obtain recipes to cook with. The diet is life long.
5. To avoid repeated biopsies to find out if this baby sometimes receives
gluten, one may also measure the anti-gliadin antibodies in the baby.
These can be used to follow if somone if following the diet, I imagine it
could be used in this case from time to time to determine if the baby is
exposed to gluten via mother's milk
I am curious as to how the diagnosis was made in this baby. Celiac
disease is rare to start with and even rarer in breastfed infants. This
is very interesting.
Let me know how the baby is doing and if you can find out any additional
information.
