Hello Pam,
I have an article by Heather Harris RM, IBCLC entitled "Remedial Co-Bathing
for Breastfeeding Difficulties" from Breastfeeding Review, Vol II, No. 10,
Nov 94, pp 465-468.  It describes this process with nice photos.  It seems
geared to the early newborn period though.  I did not see anything to address
older babies.  I believe this is an Australian journal, as someone from
Australia sent it to me.  At the end of the article is the address of the
author, you can private e-mail if you want it.  Maybe someone else will know
how to retrieve this article quickly.  I have no personal experience with
this process.
Also I wanted to point out that if a baby is not gaining well like the one
you describe and had lost a lb in 2 wks, besides bf mgmt be sure to look for
underlying infection (UTI - which was found in this babe's case), heart
problems, inborn error of metabolism, etc.
This was looked for in your babe's case, but I just wanted to re-emphasize
it.  Sometimes the bf if focused on and something else missed.
