Chris, it's probably too late, and you've probably tried this, but here goes....

I would line this baby's nose up with mother's nipple.  This means sliding him
further round the mother's body, and thinking in terms of getting the chin onto
the breast first - he will open wide.  I find mothers see the point when you
explain that their nipple needs to be in the top third of the baby's mouth to
allow lots of room for the tongue to work under the nipple.....

Get the mother to pull the baby's body and shoulders towards her, not the head -
then the chin will make contact first.  I am probably not making this clear - I
think 'Bestfeeding' by Fisher, Renfrew and Arms is available in US, and is very
good on this.

I have seen very good results with this approach.  Worth a try?
