Unfortunately there are no data available concerning the milk
levels of Molindone. Molindone is a rather unique structure, but provides
effects similar to the phenothiazines, butyrophenones, thioxanthenes. It
has both antidepressant and antischizophrenic effects.
Following a 30 mg oral dose, molindone plasma concentrations of 2
ng/ml have been detected. the plasma half-life is only 1.5 hours, but the
duration of action approaches 36 hrs, suggesting a large volume of
distribution in deeper compartments. The fact that the plasma levels are
so small is fortuitous, since it is possible the amount transferred to
milk would be in the 2-4 ng/ml of milk range. This is just a guess !!!!
However, this product appears to produce unusual endocrine effects
in women. Moderate engorgement of the breast with lactation has been
reported, along with menstrual irregularities, decreased libido,
gynecomastia(males). These effects are also somewhat typical of the
phenothiazine tranquilizers and may indicate increase prolactin
production(not that this is a problem, just interesting).
Extrapyramidal symptoms, and tardive dyskinesia have been reported.
So in essence, I can't advise you on the safety of this product in
lactating women. If it is being used solely for its antidepressant effect,
I would suggest change to one of the SSRIs(sertraline) which as a family
are much safer.
If used, observe infant for extrpyramidal symptoms, sedation,
hypotension, etc.
