I also tend more toward cold therapy for severe edema associated with the
engorgement phase of lactogenesis. Heat increases metabolic rate, perception
of pain, and increases swelling.
You know you've been to too many lactation seminars when you cant even recall
where or when you saw a presentation, but... some years ago now I saw Sharon
Humenick present on assessment for engorgement. My recollection is that
there was some sort of water displacement test. The other vague recollection
is that when they tested this empirical info against a mother's self-assessed
report (scaled with numbers from least to most engorged) they found the
mother's report to be reliable. I assume this lead to that scale which Hill
and Humenick used in their very nice studies in JHL a few years ago. I'll
try to look up the original research when I go into the office next week.
