One possible cause for the scab with clear yellow fluid in your question
#3 is impetigo.  Since this mom has a 4+ year old, maybe the child has a
case or the mom has a skin condition such as dermatitis which got infected.
I'm not sure how impetigo would explain a more prominent nipple, unless
there's some swelling as well.  She should probably see a physician for a
