I would look at two things with this baby:
1. Foremilk/hindmilk imbalance: Maybe when the mother is pumping she has
trouble getting MER and doesn't get much hindmilk (or isn't pumping long
enough to get it). Same thing could be happening while nursing (inhibited
MER) or she is taking baby off the breast too soon. Babies getting too much
foremilk are often extremely gassy and pass very runny BMs which can be
green and explosive.

2. Dairy intolerance, or perhaps intolerance to another food (I noticed
peanuts mentioned in another post). Mum may need to do an elimination of
dairy to see if this makes a difference. I once had a client whose baby's
abdomen inflated dramatically and who quit breathing on her (started
breathing again after mother shook her and baby vomited). Mother had had
tons of milk the evening before this happened. When she eliminated dairy,
everything was fine--no repeat of the scary incident,and also no more gassy
baby. Older child also had trouble with milk.
