Unfortunately, I have no experience with prozac and breastfeeding, but
I'll take a stab at this with my newly purchased (at LLLI Physician's
Seminar) copy of Briggs. They cite one case report in a woman on 20 mg
daily whose plasma and breast milk were checked, showing a milk:plasma
ratio of 0.29 for fluoxetine and 0.21 for its active metabolite, 2 months
after starting therapy. No adverse effects were noted in the baby, except
for irritability in the first two weeks of treatment. The AAP has placed
all antidepressants on its list of medications which are to be used
caution because long-term effects are unknown. My gut feeling about
prozac is that its extremely long half-life may make it less desirable
than another antidepressant, because it takes so long for it to get out
of the mother's system in case of adverse reactions. So if another
antidepressant would work in this mother's case, it would probably be
preferable, although they all fit into the "use with caution due to
unknown long-term effects" category. Hope this helps. Alicia.
