Dear Cheryl,
I know most of the experts are now in Scottsdale (wish I were :(),
so I'll add my two cents worth about your failure to thrive baby.
I've worked with BF moms for 24 years (mostly as a LLL leader)
and recently had my 1st mom with insufficient milk supply.  Have
you asked mom the questions about breast changes during pregnancy
and after the baby's birth?  I'm not sure about this, but could
an occupational therapist or speech therapist check to see whether
there's anything going on in the mouth that you or I wouldn't
catch? The last thing is something that has been talked about
before--cranial massage by a knowledgable practitioner.  Could be
something is out of whack.  In Ohio we've had several cases where
cranial massage did miraculous things for "bad" suck.
