Regarding your note about delayed milk production, as I read it, I felt
shivers down my spine; you described almost perfectly a client I worked with
about 3 months ago.
Baby latched on well but after a few minutes of sucking vigorously, no
swallowing heard, and he would pull off agitated.  Mom began double pumping
with a hospital grade electric pump with minimal results.  She took a 10 day
course of Reglan (10mg TID), also other herbal lactogouges (teas, brewers
yeast).  She pumped 6-8 times per day for 1 month with continued minimal
results.  She never established a milk supply.  I had suggested talking with
her physician about hormone tests (thyroid), but I think she didn't want to
invest anything more emotionally into the situation.  I also tried to
persuade her to use a supplemental tube system at the breast, but again she
declined.  At one month she returned the pump and never experienced any
engorgement, even though she pretty much quit pumping "cold turkey".
Needless to say I was stumped, and never turned up any other information from
local consultants I discussed the case with.  I guess I would suggest the two
things I wanted this client to try (supplemental tube at the breast, and
hormone studies) with your client.
Keep us posted - perhaps it will shed some light on the case I was involved
with.
