A possible solution to all those who in fear and trembling discharge infants
to inexperienced moms at 24hrs of age.  At the hosp in Miami where I worked
as an LC,  I wrote a Breastfeeding Care Map to assist the nursing staff in
"covering all the ground" in a limited time frame.  It was patterned after
Marsha Walker's with some changes.  Peds and OBs  were invited to a committee
meeting where the care maps for all the OB patients were revised and
critiqued.  Out of this committee came a proposal to include in the managed
care package a home visit by an OB nurse between 3-5 days of age.  The OB
nurse was expected to assess mom and infant and refer to the LCs if there
were problems.  This again was part of the patient's managed care package
therefore no additional charge for this service.  It was the only way our
Peds would allow 24hr D/C to a breastfeeding primip.  The other option for
the hospital was to eat the cost of another days stay.--Diane
