     Received a call from mom with 7 week old exclusively breastfed until
yesterday afternoon.  First talked to her at 4 weeks regarding infrequent
stools (yellow typical) and appeared to be straining with gas at other times.
 Fed every 2 hours X 15 min. during day and more frequent at noc with
fussiness.  He had been gaining weight and growing fine.  We discussed diet
in regards to gas and fussiness...there is a family hx of lactose intolerance
on fathers side.  He also nursed one breast per feeding because he falls
asleep (experienced this with first child also).  My first thought was a need
to increase feedings to both breasts.  Second, to watch dairy intake.
     First peds said he had tight rectal muscles and to digitally dilate
daily to assist with expulsion.
     She just went to another peds that felt there was a milk intolerance and
to "experiment" by giving him exclusive lactose free formula for atleast 3-4
days and note any changes.  She cut down her dairy intake since last talking
to me, but still has cheese, yogurt and ice cream...cooks with butter.  Still
nurses 1 breast during day...but, the other evening (which is the only 3 hour
pereiod he seems fussy) he took both sides and slept atleast 7 hours.
 Yesterday afternoon he started ABM 4-5 ounces and was pretty good.  She
called me this morning regarding the peds info and strong desire to
continuing nursing.  I don't feel he would have less than 12 hour cure from
"lactose-free" food.  The fact that he obtained more food each feeding from
bottle was probably providing more saitiation of appetite/calories.
     Please offer words of wisdom...am I on the right track?....If
lactose-intolerant what advice for moms diet.....do lactaid tablets help only
mom and not baby through breast milk....
      It just doesn"t sound like an all day occurance and more like need for
increase feeding time duriong day.  Fussiness at night if fed well can be
baby typical also.   Hope I made sense..gotta run to work and my ten year old
wants the computer!!!!
