What a stressful situation for your grandchild and everyone in the home!  

I very much appreciated reading the responses to your post thus far.  

I will reinforce the need for a GI work-up, and feel it is necessary to rule out reflux/GERD.  Reflux may be occurring more so at night since the baby is presumably laying flat in bed without any help from gravity to help keep the milk and food in the stomach, as when held in a more upright position.  Holding babies with reflux more upright during a feed and also for approximately 30 minutes after feeding are standard GERD recommendations.  https://www.healthychildren.org/English/tips-tools/ask-the-pediatrician/Pages/What-is-the-safest-sleep-solution-for-my-baby-with-reflux.aspx

When your granddaughter appears to be in pain re: crying and arching and throwing her head back, these behaviors may also be clinical signs of reflux in regard to the baby appearing to be trying to move away from the pain of reflux.  If GERD is diagnosed, her esophagus may be quite irritated, since these difficulties have been occurring for a while now.  

Although reflux boards were once recommended for many babies with reflux (to keep the baby's head more elevated than the rest of their body, utilizing gravity to keep the milk in the stomach and out of the esophagus), the American Academy of Pediatrics no longer recommends reflux boards or other gadgets to keep the baby more upright, in recognition of safe infant sleep practices.      

Does your granddaughter have more chest and/or nasal congestion when she is put down, and is she less congested when held upright?  This information would also help the pediatrician to rule out GERD.
