Loretta, the mother you mentioned was being treated with antibacterial and antifungal medications, unsuccessfully, and you asked what had been overlooked. It looks to me that mechanical causes have been overlooked, for which the first step should be attention to latch and the infant's position in relation to the mother's body.
 It may well be that the medications didn't work because the cause of pain was mechanical. Only after the mother has been assisted to find a comfortable latch, and learned not to lean forward - i.e. mechanical factors - can medications be considered. The fact that they didn't work suggest to me cthat they weren't necessary.
