Well said Tricia, about what did we do before quantifying everything and being able to pump, like that shows someone who is only pumping to see her milk supply what her milk supply really is from day to day. 
But your letter made me think of the book My Child Won’t Eat   By Dr. Carlos González. I read it so long ago I don’t remember if there was research or more of his observations as a pediatrician, but he faulted all those 8 oz bottles for limiting hunger and interest in solid food.
