Hi - the article "Revisiting Nipple and Breast Pain: A Conversation With Anne Eglash, MD" is really interesting and discusses a biofilm / crust may give you some ideas.  It is in Clinical Lactation, Volume 6, Number 3, 2015, pp. 124-128(5).
