Amy, I am with you in following a manufacturer's instructions. IF a case was called in to court of low milk or low weight gain with medela shield use, the nurse or LC would not have a leg to stand on if manufacturer's instructions had not been followed. But most of all, I believe it is best lactation management to give a mother the best opportunity for making a plentiful milk supply. Susan in Minnesotarn ibclc
