This has been a great and detailed discussion.

Our local, non-BFI hospital, will give anyone a shield that has 'flat'
nipples and use the 'ram and jam' to force the baby onto the breast. Maybe
if they taught appropriate latching and positioning, moms wouldn't have
nipple pain?

When I see clients, I spend most of talking to them about how breastfeeding
issues were not caused by her because her nipple is flat (rarely, is it
really flat) and it's normal for milk to not come in in 12 hours. It's
infuriating.

I have all sizes of mamavac shields in my office. I give them out with
instructions as needed. Half of those uses have been to get the baby off
the bottle and back to breast. 1/4 to heal an injured nipple and 1/4
because mom has anxiety with the baby latching onto her skin (I have
counselling background, so we talk a lot first).

I always instruct to use compressions while nursing with a shield and to
hand express after, to maximize her supply, like with the pumping protocol.
It's up to mom to follow through with that. Most do the compressions and
hand expression, but stop the hand expression after a few days because
their milk rebounded nicely.
