Subject: thick crust on nipples antenatal

PTP:  Mom is 36 weeks pregnant, has Lupus and hypo or hyper thyroid (she can't remember which).  she noticed this small crust on her nipples about 1 year ago before she was pregnant.  Since then it has gotten worse and covers the entire face of the nipple on both sides, worse on left side.  The "crust" can be picked off and per mother has the texture of dried glue, kind of gummy if you will.  When she picks part of it off it is a bit sensitive underneath and it comes back after picking it off.  It is not painful and does not itch.  She has tried moisturizing it with oils and creams and it becomes softer and the color changes more to nipple color but it does not go away and it just dries up again if not moisturized.  She has asked her OB about it but he has not examined the nipple itself.  He prescribed an antifungal, which mother used for a few days with no improvement.  She will push her OB more to actually take a look at it.  She is concerned that it may block nipple pores, and that the baby will suck it off and then her nipples will be more sensitive/sore when he latches.  I took a picture of it for reference, but I don't think I am able to post pictures here.  If anyone thinks they may have seen this before email me and I'd be happy to send you the picture in order to help this mama figure out what her body is doing.  TIA
