Mother's anatomy absolutely must be taken into account. I have seen nipples damaged from shoving them into too-small of a nipple shield. In addition, if the nipple is too crammed into the shield, the nipple pores can actually be occluded, interfering with milk flow. I often try a couple of sizes to see which works best for the mother and baby as a whole.  Sometimes what allows the milk to flow is on the large size for baby, but what is the point of sizing the shield to baby if milk cannot flow?
