Do you have available the smaller 21 mm flanges?

Subject: Assisting mothers after weight loss surgery
Hi everyone,
I have seen several mothers lately that have had weight loss surgery. The mothers have difficult pumping due to loose skin on the breast. The tissue is very stretchy and the entire areola along with skin is pulled into the "tunnel" of the flange/horn. Changing sizes does not eliminate the problem. The facility in which I work uses only Medela Symphony pumps. Most moms are able to obtain a pump (most have several brands to choose from) through private insurance or WIC.
I'm wondering if anyone has seen this as well and if so, any suggestions on how to make pumping more comfortable for these women?
Thank you in advance for your wisdom,
Kristi Downs
