Dear Pat,
I am saddened by the article. I find that having a tight frenulum 
released can help breastfeeding tremendously. We do need to be making 
sure that we are working on better latching and positioning first 
(thanks Nancy Mohrbacher for reminding us of this), all of the things we 
do as good IBCLCs. BUT, if over a week with good breastfeeding 
management, and the baby presents with a possible tie, having the baby 
diagnosed by a qualified practitioner is warranted, in my opinion, and 
if that specialist feels the evidence justifies a revision, doing it 
helps improve breastfeeding 95% of the time.
This is my clinical experience.
