Women's Breast Health Survey Title of study: Epidemiological study of female healthcare professionals who have taken chloroquine
Purpose of the study: Assess the prevalence of breast cancer among women who have taken chloroquine for prevention of malaria. 
Sponsor: George Mason University
Investigators: Virginia Espina and Lance Liotta
Scientific methods and procedures: Anonymous health questionnaire will be distributed to participants.  Female healthcare professionals who self-identify as having taken chloroquine in the past or are currently taking chloroquine for prevention of malaria AND also those who have NOT taken chloroquine for the prevention of malaria are all eligible to participate in the study. 
Anticipated risks to the participant: There are no anticipated risks to the participant other than the time burden of completing this questionnaire, which is estimated to take 15 minutes. No identifiable data will be collected now or in the future. There is no significant risk of disclosure of personal information because the questionnaire results are submitted back to GMU scientists without personal identifiers.
No direct benefits to the participant are expected or implied. You will not be compensated in any form for participation in this study. The potential benefit of this epidemiologic research is societal in nature, such as obtaining new information about potential chemoprevention drugs.
