You could consider an SSRI such as fluoxetine or sertraline. These can help premature ejaculation in males, and sometimes make orgasm more challenging for women.

Subject: Orgasm during feeds
Hi,
I am writting on behalf of a Spanish colleague.
She is working with a mother who is expecting her second child.
She weaned her first baby a week after birth, never telling anyone why. Now
she has explained to my colleague that th reason she weaned abruptly was
because she was having orgasms with the baby´s suckling and it was not an
acceptable situation for her.
Now she is expecting her second chld and would like to breast feed, but
fears the same situation.
ANy ideas on how to handle this? I´ve proposed that, if it happens again,
maybe she can try nipple shields and see if there is less stimulation (and
if baby can managa to feed well with them, of course). Or maybe express
milk and feed it to baby instead of direct breastfeeding...
