Thank you Jan!    as the only LC in a community hospital, I absolutely expect the nurses to give shields.  I will agree, they have
given them on occasion when not necessary and by discharge, Mom is not using.
I have educated the staff on proper instruction of shield use as well.  Same, I have seen Moms that are not applying correctly.
I just had a scenario this week, flat nipple and inverted nipple.  After birth, baby did skin to skin and licked at breast but unable to sustain a latch. Mom and I unable to express more than a drop of colostrum.     I  gave this Mom a nipple shield
formula / bottles began being mentioned by the family.    If you can hear swallowing with the shield, observe output and give instruction of breast massage, pumping before to attempt latch without a shield I do not see a problem.  Follow up post discharge is
important as well.
