Subject: Baby vomiting after breast or EBM but accepting artificial formula
I shall be seeing a mother who reports that her 2-week-old baby is vomiting
after breastfeeding and after drinking EBM by bottle. She reports her baby
keeps down and "likes" artificial formula by bottle. Obviously I haven't
seen them yet and so I haven't observed the situation, and when I do I'll
be looking for factors related to what and why.
In the meantime, I'd be grateful for some suggestions out of left field.
It's the vomiting of breastmillk (however it is given) and not after an
artificial product that has me puzzled.

I've seen/heard of this before, in a couple cases. I had no opportunity for follow-up, so I don't know what ultimately happened.
My guess is that it's something the mother is eating that the baby is sensitive to. In one of these cases, the milk the baby spit up was from the mother's cousin, who was also a milk bank donor. The baby did not spit up after his mother's milk or after artificial milk, only after the cousin's milk. With this mother and baby, I would go through the usual eliminations: wheat, non-human milk, soy, shellfish or fish, or a medication or vitamin the mother is taking. I would tend to doubt corn because, although it is a common allergen, it is a common ingredient in artificial milk and the baby is tolerating it. So, this mother might want to do an elimination diet for a week or two to see what happens.
I seem to recall Dr. Jack Newman saying that some spit up after breastfeeding may be beneficial because it gives the esophagus a second chance to get coated with milk. How much vomit are we talking about here? The whole meal or a little burp up?
