hope some one has seen this and has some ideas
I have permission to share the mother's history - observations
29 yo 3rd time mom - nursed #1 for 3 weeks - #2 pumped and bottle fed for 9 months
HX - as soon as get pregnant her nipples begin to "slough off" - skin peels around nipple
#1 went to Dermalogist and was told nipples would not heal as long as there was milk - quit - did not know what it is
#2 Culture nipples on 3 occassions - first time is was only skin - staph epi the other 2 times did not grow Yeast or any bacteria. Tried OTC antibiotic - cortisone - nothing really helped so just pumped

#3 pain and redness started 5 days after missed period - by 6 weeks areola skin was peeling
Baby has nursed twice - there is NO problem with latch that can be fixed
The nipple is erect without color change - no obvious damage to the nipple
Both areola are bright red -
Parts look like a sunburn - where the skin is peeling (not flakey) the skin is raw - angry red
where the skin is intact and not peeling there are cluster of pin point yellow pimples - look like grape cluster - there are 4-5 of the pimples all in long cluster - peeling skin then more cluster
They are more prominient on the top of the areola but appear scattered throughtout
She says her areola "burns"
It looks like a vaginal yeast infection - that red - but there is no evidence of yeast
Hydro gel pads are soothing but does not heal

Her goal
1. She would like to know if anyone has heard or seen anything like this
2. Limit the damage so that she does not get mastitis - she had 3 times with baby #2
Is going to use antibiotic ointment and 1% cortisone and gel pads

She wants to breastfeed but will pump and feed with bottle if needs to
Warm tea bags are soothing but does nothing to heal

Nipple Shield?
Has anyone used Burrow's Solution? Aluminium Acetate that is an astringent and antibacterial - trade name Domeboro

Thanks in advance
