Kudos for initiating more awareness on insufficient milk supply! Because you've asked for comments, here are mine:
I know that this article is for the lay public, so it is best to not keep it too complicated. Your blog post addresses one type of mammary organ failure, which is hormonal. Often this is due to insulin resistance, high androgens, and I am often wondering if receiving estrogen and progesterone during the first trimester is having a negative effect on breast development during pregnancy. Usually this is not due to low prolactin levels, unless mom has had brain irradiation, traumatic head injury, or inflammation of the pituitary gland.
I didn't see another large population mentioned, which is the insufficient glandular tissue group. These women are often quite healthy, not obese, no insulin resistance, but just have an insufficient amount of glandular tissue. They tend to have high prolactin levels. Domperidone does help this group, but I find that time and goats rue are the most helpful.
In general, there are many causes for heart failure, kidney failure, liver failure, and other organ failures. We can keep this in mind for mammary failure as well.
