"Milk under the skin…a trivial condition only mentioned for completeness. It is not common. In it milk apparently gets under a small area of the epithelium of the nipple, making a small white bleb, usually a triangle with 2mm sides. It can last for some days, usually cures itself by the peeling off of the lifted epithelium, can sometimes be helped by removing this with sterile precautions, and is best treated by aqueous crystal violet paint to keep away fungal infections [And inhibit skin staph too: MM]. While it lasts feeding from the affected nipple hurts."  Infant Feeding (Methuen &Co 1970) p. 84.

Personally I used a sterile needle to puncture the outer layer, and trimming the epithelium was sometimes necessary to prevent recurrence. (The exposed under layers were incredibly sensitive of course, all those unmyelinated nerve endings. But when doing this to oneself, one can feel where not to go!)
