The new core measure (PC-05a) "Exclusive Breastmilk Feeding Considering Mother's Initial feeding Plan"  requires us to give mom a letter stating the benefits of breastfeeding and risks of formula that she is supposed to read and sign and we have to scan it into baby's chart before the first feeding takes place. Is anyone else doing this?  If yes, would you be willing to share a sample of your letter or share what is included in the letter and the wording used?  Are there any sample letters on the internet somewhere?  Thanks!!
