The required documentation outlined below regarding benefits of  
breastfeeding/hazards of formula is necessary and beneficial to moms  and families, 
*breastfeeding this time around or not*, ... and is the most  likely item to 
keep everyone out of court in those rare instances when that  action might be 
later threatened (just one of the reasons it's all so  complicated)
 
Having the mom _Sign_ those and related types of  documents, and retain a 
hard or soft copy of each, helps the mom  know/feel/internalize that she has 
been informed and educated, and she, by  signing, is acknowledging and 
taking responsibility for now being educated  and informed, and for her feeding 
choices and decisions, which short and  long term have great potential to 
effect the health of her baby and of  herself.
 
Let Joint Commission be impressed (in this way and other ways) that astute  
and insightful health care providers went above and beyond minimal  
expectations.

Our  hospital was unable to find anything written in the Joint Commission's 
 guidelines that specifically states that the mother must sign the  
document.  The only requirement was that the practitioner document the  date/time 
of the discussion w/ the mother in the mother's chart.  Our  Pediatric team 
then makes note in the newborn's chart that they viewed  documentation of 
this discussion in the mother's chart.

Anyone else  find otherwise?
