Hi Lisa,

A very unfortunate outcome, for sure. Here are my words of wisdom:

1. Keep all documentation
2. Write down any additional facts about the case that you can remember,
like your conversation with your superiors
3. Have some relevant articles at hand like this one on hypernatremic
dehydration related to (ineffective) bf

4. If you have time, look into whether your privacy rights were violated
and report that to relevant authorities; not sure how to find this out: CWS
or HIPAA (are you in the USA?) violations; are you in hospital? Your risk
management department or social services may know something about this
5. Do not let this deter you from reporting in the future

Sorry this happened and this shows why some people are reluctant to report.
However I do believe the infant was at risk of permanent disability or even
death and I know you did the right thing.
