I certainly understand you being upset with this Pediatrician.  However, I would approach him (or her) a little more non confrontational.  Perhaps.. "the patient (who is also a fantastic peer counselor) understood what was said as........and although I am certain that is not what you meant, this patient is very confused"...and since she is assisting other breastfeeding mothers you would appreciate his (or her) input (and years of wisdom) in helping this mom understand what you (the MD) meant.  I know this is rather patriarchal, but maybe by wording it so that the physician can better educate this patient (and others...and him or herself)   he or she might learn from the situation...we can hope.
You can tag your references in the comments that you know s/he are aware of...
Maybe there was some misunderstanding in what was said, I highly doubt it, but kindness and flattery go along way...still in 2014!
Good luck and keep us posted!  I do hope that this  mother IS NOT discouraged, continues to breastfeed her little guy as long as he wants to and that she continues to be an excellent peer counselor for others~
