Sounds like what you might be describing is scar tissue. I would not
recommend breast pump use at this time. But rather gentle reverse pressure
softening (RPS) to see if colostrum will release. I think you should
certainly be able to hand express colostrum if the baby is able to
transfer, especially at the end of a bf session. You are right to monitor
baby closely and that he may not be transferring any milk at all.
