We do not have such a form, but would be very interested in seeing formula consent forms others use. Please post to the group, or send forms to me as well.

I am wondering if those of you based in the hospital setting are using a consent form (similar to the form to use donor breast milk)for formula supplementation if the mother doesn't want to use donor milk? We are in the final stages of being able to offer donor milk to not only our NICU patients, but to newborns on the post partum unit who have risk factors.
